Campus Dining and Shops/Three Pillars Catering continues to explore methods to improve and maintain a safe and healthy work environment.  This is one method you can use to help us to identify potential safety concerns in addition to reaching out directly to your manager, Assistant Dining Director or Director, another member of management, or contact human resources.  The goal of this reporting form is for safety concerns to be reported quickly and efficiently which will lead to faster investigations into concerns and swift actions to mitigate risk.  Safety in the workplace is our priority and we want to continuously improve safety practices, procedures, and training for our employees.

Individuals can report concerns through this portal related to safety issues in the workplace including but not limited to tripping hazards, broken or failed equipment, unsafe actions or conditions you witnessed, or other concerns related to workplace safety.  After submitting a report, a user will receive a confirmation that the report has been submitted and if contact information is included, you will receive a follow up related to your report.  While we encourage you include your contact information so that we can ask any additional follow up questions to assist in investigating the concerns, anonymous reports are also accepted.

As a reminder, there are several ways to raise a safety concern:

  1. Discuss the issue with your immediate Supervisor or Manager
  2. Discuss the issue with your Assistant Dining Director or Director, another member of management, or contact Human Resources
  3. Discuss with a member of the Safety Committee
  4. Submit a safety suggestion or concern in the safety box outside of the Human Resources office at 175 Richmond
